Tuition Payment Agreement

I agree to pay the LEB for the dance instruction of the above student per the published tuition rates for the student’s period of study, unless I am applying for 1) Financial Aid (sliding scale tuition rates) or 2) Scholarship. I understand that I can make a payment by check, payable to “Lake Erie Ballet”, MasterCard, Visa, or cash, and that there will be a $20 charge for returned checks and declined credit cards.

I understand that payment of tuition entitles students to take all the scheduled classes at their level, and that no refunds are given for classes missed because of illness or vacation.

I understand that tuition fees are due on the first of each month or the first of each semester; that if installment payments have not been received by the 10th day of the month or the 10th day from the beginning of a semester, a finance charge of 5% per month will be assessed as of the due date.

Sliding Scale Tuition Policies

1.)	All registrants applying for sliding scale tuition fees must provide copies of the household tax return from the current year by the required deadline.
2.)	Registration for classes is seasonal. Once tuition fees are determined, the fee will stand for the duration of the season as long as students are enrolled and attending classes. Should enrollment change from term to term, the revised tuition fee will reflect the scale within which applicant applies and has been accepted.
3.)	All tuition is due before classes begin.
4.)	There will be no pro-ration of tuition for absence from classes.
5.)	Installments are due by the 10th of each month and are non refundable.
6.)	A late fee of $10 will be assessed on all accounts over seven days delinquent. The account will be charged an additional $10 each week there after until account is paid in full.
7.)	Unpaid or delinquent accounts will amount to restriction from classes after one month of delinquency.

Financial Assistance/Scholarship Deadlines:

• Term I, August 28
• Term II, January 15
• Term III, May 25

Financial Assistance (sliding scale tuition rates), Work/Study Exchange and/or Scholarships Agreements are assessed each Term. Once arrangements have been made, agreements will be honored for the duration of the season during which the agreement is made. Please email the Lake Erie Academy of Dance if you have questions.

	Non-Student $15.00 (1.5 hrs.) or $12.00 (1 hr.)
	LEAD Student $12.00 (1.5 hrs) or $10.00 (1hr.)

Class cards:	4/1.5 hr, 6/1hr, or 8/45 min. classes
	Non-Student Any Level:	$60
	LEAD Student Advanced:	$32
	LEAD Intermediate:	$40
	LEAD Beginning Or Recreational:	$48
	LEAD Children's Division:	$60
